AMDRAS Renewal Form

We are now under the AMDRAS Standards. Use this form to renew your National Accreditation if you are a current Mediation Institute Member

Are you a nationally accredited mediator and ALSO an FDR Practitioner or FGC Facilitator? Use this form.

If you are not a nationally accredited mediator or are accredited through another Recognised Accreditation Provider (RAP) use the Practitioner Member renewal form.

Practitioner Membership Renewal Form

Renewal for FDRP's, FGC's and NMAS/AMDRAS Accredited mediators not accredited through Mediation Insitute.

DO NOT USE THIS FORM - if you are a NMAS / AMDRAS Mediator accredited through Mediation Institute. Use the NMAS or AMDRAS form even if you are also a FDRP or FGC Facilitator.

Transfer to Mediation Institute

Use this form if you are a NMAS / AMDRAS Accredited Mediator accredited through another provider and you would like to TRANSFER your accreditation provider to Mediation Institute.

If you want to stay with your current accreditation provider but be a practitioner member with Mediation Institute use the Practitioner Membership Form.

Leave of Absence or Non-Practicing Certificate Request

Use this form if you are not currently practicing and want to request some time out:

Leave of Absence are for up to 1 year.
Non-Practicing Certificates 2 years, renewable.

Accreditation Reinstatement Form

Use this form if you were a Mediation Institute accredited Mediator NMAS / AMDRAS and your accreditation has lapsed.
