Are you a nationally accredited mediator and ALSO an FDR Practitioner or FGC Facilitator? Use this form.

If you are not a nationally accredited mediator or are accredited through another Recognised Accreditation Provider (RAP) use the Practitioner Member renewal form.

Renewal for FDRP's, FGC's and AMDRAS Accredited mediators not accredited through Mediation Institute.

DO NOT USE THIS FORM - if you are an AMDRAS Mediator accredited through Mediation Institute. Use the AMDRAS form even if you are also a FDRP or FGC Facilitator.

Use this form if you are an AMDRAS Accredited Mediator accredited through another provider and you would like to TRANSFER your accreditation provider to Mediation Institute.

If you want to stay with your current accreditation provider but be a practitioner member with Mediation Institute use the Practitioner Membership Form.

Use this form if you are not currently practicing and want to request some time out:

• Leave of Absence are for up to 1 year. 
• Non-Practicing Certificates 2 years, renewable.

Use this form if you were a Mediation Institute accredited Mediator AMDRAS and your accreditation has lapsed.

Use this form if you are a Mediation Institute AMDRAS Accredited Mediator Member who is now eligible for Advanced Mediator Accreditation. See the form for the full details of the information you must provide to support your application.
Use the Renewal form if you are within 60 days of your AMDRAS renewal.
